The 1–5 Scale I Use With Health Coaching Clients

A symptom scale is not just a number. It is a way to communicate.

A symptom scale is not just a number. It is a way to communicate.

Most people have been asked some version of this question: “What is your pain on a scale of 1 to 10?”

But this kind of scale is often too confusing. A patient may say their pain is a 12 because they feel like they can no longer function. A doctor may hear that number differently. That gap can make people feel dismissed, misunderstood, or rushed into the wrong solution.

In this episode, I explain why health scales are really a grammar for communication.

I also walk through the 1–5 energy scale I use with health coaching clients:

  • A 1 means your energy is so low that you can barely get out of bed.

  • A 3 means your energy is steady and functional for most of your day.

  • A 4 means you have extra energy to spare, but you may crash if you stay there too long.

  • A 5 is not the goal. It can feel jittery, scattered, and unsustainable.

This matters because many people say their energy is “bad,” but they have lost the ability to sense what their baseline really is.

When you describe your symptoms more clearly, your doctor, health coach, dietitian, nutritionist, or trainer can help you more effectively.

The goal is to understand what your body is actually trying to tell you.
